It looks like recently shortened nagamaki-naoshi. The shortening was performed in late 19th - early 20th century (or even more recently, it's hard to judge). Additional mekugi holes were filled in non-traditional way, either as a hasty job or by a non-Japanese specialist. Polishing looks recent and looks like it was done by an amateur - geometry is a bit off, nicks are rust spots were left on the blade.

I can't really tell much about smith/age/school of this blade because it was altered recently. Most likely it's a Shinshinto sword.
